Waiver

     The Home and Community Based Services Waiver Program provides specialized services in the home as an option to nursing home placement for Medicaid eligible clients requiring nursing home level of care.

 

     Services offered under HCBS are case management, respite care in home or nursing home, non-medical transportation, adult day care, home-delivered meals, personal care attendant and personal emergency response system.

HOW TO APPLY:  Call office to set up assessment visit; client will also need to make an application for Medicaid through Department of Family Services.
